WHAT IT IS
As a member the ANBLPN, you have a no-cost $4,000 Accidental Death & Dismemberment benefit. This AD&D benefit and other items listed below are provided jointly through ANBLPN and American Income Life Insurance Company (AIL).

HOW TO RECEIVE YOUR CERTIFICATE OF COVERAGE
Notices are sent out annually at the beginning of the year; if you do not receive yours, it is likely due to inaccurate address information in your account.

When you receive your notice, you will have the following options:

1. Complete the beneficiary card to designate your beneficiary and return it to AIL using the postage-paid return envelope. OR
2. Respond online at mybenefits.ailife.com using the access code provided in the letter you received in the mail.

When you complete one of the two options above, you will receive a certificate of coverage with your beneficiary information. If you do not name a beneficiary, the benefit would be paid to your estate and may be taxable.
